Comprehensive Care Management: Keeping you Safe, Healthy and Connected

We help you navigate the obstacles and challenges of staying independent, safe, and healthy. Together we problem solve and connect you to resources for home health, basic needs, and housing resources. We help you to prevent or manage health changes and medical challenges. We can help you find food to supplement your benefits, social connection to fight loneliness, mental health support, and any other unique challenges you face.

The following are examples of ways we can help.

Keeping you SAFE

  • Securing appropriate housing and shelter
  • Domestic violence intervention and support
  • Helping you communicate in challenging or complex situations
  • Responding to antisemitism or bias

Keeping you HEALTHY

  • Applying for government benefits like SNAP, TANF, disability, affordable housing and more
  • Crisis intervention and assessment for mental health issues
  • Transportation support for medical appointments and other urgent needs
  • Finding appropriate home health care during a medical crisis or change in your support system

Keeping you CONNECTED

  • Finding support groups, clubs or other activities to find friends and community
  • Creating new clubs and gatherings 
  • Referring you to our community statewide calendar and other organizations
  • Offering support groups to improve your coping and well being

Help Coping with Antisemitism

If you have been a witness or victim of an incident, follow our guidelines for reporting and support. Call any of our case managers for more help. We offer help with reporting and emotional support. 

Community Gatherings

Social isolation is a leading health risk of our time. We can connect you to our Book Club, Movie Club and more. Do you have an idea for a new one? Call us!

Support Groups

We routinely offer a variety of support groups for Caregivers, Domestic Violence Survivors, Grief, Depression, Aging and more.

Connecting to Other Resources

Our community care managers (case managers) can help connect you to other programs that provide housing, food assistance, and more. After your first meeting with us, together we figure out which resources fit your needs.

Phone Friends

JCSH volunteers will call those who may be homebound or feeling lonely and would enjoy chatting on the phone. If you would like to have a phone friend - please fill out our contact form!
